Shady Grove Fertility Rockville is where Shady Grove Fertility (SGF) began: Drs. Michael J. Levy and Arthur Sagoskin opened the practice's first office in Rockville, Maryland in 1991. SGF has since grown into a multi-state network, is a founding partner of US Fertility, and keeps its headquarters campus in Rockville.

The office occupies the 4th and 5th floors of 9601 Blackwell Road. It houses SGF's largest IVF and embryology laboratory and an ambulatory surgery center, and some procedures are also done at SGF's Towson and Fairfax sites. The physician team includes several reproductive endocrinologists and a reproductive urologist who directs SGF's male fertility care.

Services include fertility and ovarian reserve testing, semen analysis, HSG, IUI and IVF. The office also offers donor egg, sperm and embryo programs, genetic screening and testing of embryos, IVF cycles with a gestational carrier, egg freezing, fertility preservation for people with cancer, and LGBTQIA+ family building, with financial counselors on site to discuss coverage.
